POPI ACT AGREEMENT AND CONSENT DECLARATION
I hereby declare and confirm that I, the person / entity / body / individual / company providing the information, hereinafter collectively referred to as the "client," do hereby hereby irrevocably agree and understand that any/all information supplied or given to the service provider (Sanca National) is done so in terms of the below terms and conditions and in terms of this agreement and consent declaration.
TERMS AND CONDITIONS

1. I agree that by registering for the #quit virtual programme, I am responsible for the full payment for all 5 sessions, even if I do not attend all of the sessions.
2. If I am unable to attend one of the sessions, I can inform the programme administrator (in advance) to re-attend the session during the next programme.
3. I will provide Sanca Sational with the totals of my Cigarete Dependence Scale (CDS) before and after the programme to measure the efficacy of the #QUIT programme (the programme administrator will clarify how this will be done once the programme has commenced).
4. During the programme, I will have access to an experienced addiction specialist if I have any personal needs.
5. If I successfully finished the # Quit programme I will be eligible for free monthly aftercare support sessions for the next 12 months.
